Insurance claim authority
Brokerage request template
A policyholder's signed authority letting a broker or representative lodge and manage an insurance claim and deal with the insurer on their behalf.
An insurance claim authority is a document in which a policyholder or claimant authorises a broker, adjuster, or representative to lodge a claim, communicate with the insurer, and act on their behalf during the claim. It records the claim or policy it relates to, what the representative is permitted to do, and the policyholder's consent to share claim and policy information. Insurers generally need this before they will discuss or progress a claim with anyone other than the policyholder.
For a brokerage or claims handler, a signed authority is what allows the claim to move forward without the insurer refusing to engage. It also captures the client's consent in writing and provides evidence of the representative's mandate if the claim is later disputed. Keeping the signed authority on file helps claims proceed smoothly and protects both parties.

In Australia, electronic signatures are generally recognised under electronic transactions legislation where the signer's intent and identity can be shown and the method is reliable. Insurers may have their own requirements, so confirm what they accept. This is general information, not legal advice.
A claimant who is not the original policyholder may need to sign, depending on the policy and the insurer's rules. Cosign lets you add whoever needs to authorise the claim as the signer. Confirm the insurer accepts an authority from that person.
Yes. The authority is sent as a private link to the named policyholder or claimant and is not indexed by search engines. Only someone with the link can open it.
Cosign tracks each open, preview, and signature, and gives you a completion receipt once it is signed. That lets you lodge or progress the claim knowing the authority is in place.
If they decline, the status updates so you can follow up and discuss their concerns before proceeding. Without a signed authority, the insurer is unlikely to engage with you on the claim.
